If you have reached this page, you are ready to submit a service complaint about the Canada Revenue Agency (CRA).
Before you submit your complaint
- Do not provide any additional documents. An officer will review your complaint and determine if more documents are needed.
How would you like to submit your complaint?
If you are submitting a complaint on behalf of someone else, please select "Mail or Fax", as you will need to fill out our complaint form, OMB100, to become an authorized representative.
Online
Our online complaint form is the fastest and most secure way to submit your complaint.
For security reasons, there is 60 minute time limit. It is recommended that you have the details of your complaint prepared before starting the application to reduce the risk of losing the information you have entered.
Privacy Notice
Personal information is collected under the authority delegated to the Ombudsperson under Order in Council P.C. 2020-0703 and is used to assess and process complaints. Information will be shared with the Canada Revenue Agency for the purposes of resolving complaints under existing information-sharing agreements and may also be used for follow-up and evaluation purposes.
Personal information is protected under the Privacy Act, and individuals have the right to access their personal information, request correction, or file a complaint to the Privacy Commissioner of Canada regarding the handling of the individual's personal information. Refer to the Taxpayers’ Ombudsperson's Personal Information Bank CRA PPU 222 on Info Source at canada.ca/cra-info-source.
