What you need to apply

You must meet all 4 of the eligibility requirements before applying.

You’ll need to provide the following information for each applicant (yourself and any dependants), and your spouse or common-law partner (if applicable):

  • Social Insurance Number (SIN) (if available for children)
  • date of birth
  • full name
  • home and mailing address
  • list of dental coverage through government social programs (if applicable)

Your spouse or common-law partner must submit their own application to be covered under the CDCP.

Providing information about your spouse or common-law partner (if applicable) in your CDCP application is only to confirm your marital status so your adjusted family net income can be calculated properly, and to verify that you do not have access to private dental insurance or coverage through your spouse or common-law partner. It does not mean that an application is being submitted for your spouse or common-law partner.

Eligibility review

We review CDCP eligibility to confirm that only people who meet the requirements receive coverage.

To be eligible for the CDCP, you can't have access to private dental insurance or coverage.

If you tell us (attest) that you do not have access to private dental insurance or coverage, we may initially accept your application. However, we continue to review applications after we accept them. If what you tell us doesn't match your T4 or T4A tax slips, you may be selected for an eligibility review.

If your application or coverage is under review at any time, we'll notify you by mail. The letter will tell you next steps and how to confirm your eligibility.

If declared ineligible

If you are declared ineligible, you will be removed from the CDCP and you may be required to reimburse the Government of Canada for any services paid for by the plan while you were ineligible.

Use My Service Canada Account

Follow these steps:

  1. sign in or create your My Service Canada Account (MSCA) profile by using the button below;
  2. once signed into MSCA, go to the "Canadian Dental Care Plan" section on your MSCA dashboard;
  3. click on "Apply for Canadian Dental Care Plan coverage" and follow the instructions

If you can't use MSCA

Apply using Canada.ca

Choose "digital" as your communications preference to receive email notifications. You can also access CDCP letters through MSCA. By signing up, you'll get important updates in one place, faster than waiting for letters in the mail.

Apply by phone

If you can't use MSCA or Canada.ca, you can call Service Canada at 1-833-537-4342 to speak to an agent. For TTY, call 1-833-677-6262.

If someone is helping you apply

You can ask someone you trust or a delegate to help you apply for the CDCP.

  • Applying with the help of someone you trust

    You can ask a trusted person to help you apply. You must be able to give clear consent that you agree to let them help you. This could be a:

    • friend
    • relative
    • caregiver
    • translator, or
    • interpreter
  • Applying through a delegate

    A delegate is someone who has the legal authority to represent you and make decisions on your behalf.

    Before you apply, your delegate must prove that they have the legal authority to represent you. They can represent you if they’re listed as your delegate on documents such as:

    • power of attorney
    • mandate
    • trusteeship

    Mail original documents or certified copies to us, or bring them in person to any Service Canada office (see mailing addresses by region).

    Once we’ve processed the documents, we’ll contact your delegate to confirm that we’ve accepted and added the documents to your file. Your delegate can then apply to the CDCP on your behalf.

    How to submit your proof of delegate documents

    By mail

    If you or your delegate will be sending us the documents by mail, please include a cover letter with:

    • your full name
    • your Social Insurance Number (SIN)
    • a statement that the documents are being submitted for the CDCP
    • your delegate's phone number, and
    • a return address, so we can return your documents for us to send the original documents or certified copies back to once we've processed them

Beware of scams

There is no fee to apply or renew your coverage for the CDCP. Be careful of scams sent by mail, phone calls, text messages, or emails that ask for your personal, banking or credit card information. Watch out for ads and pop-ups that are not from official Government of Canada websites.
