How to complete the maternity and parental leave form for federal employees

Employees requesting top-ups must select each box to indicate that they have read and understood the conditions. If you are NOT requesting top-ups, select the first box only.

Section 5: Declaration regarding other parent. Employees requesting top-ups must declare whether the other parent is a Federal Public Servant, and is also requesting top-ups. This will ensure that between both parents, the maximum number of top-ups is not exceeded.

It is important to know that your province of residence determines which governmental entity will administer your benefits. Residents of Quebec should contact the Quebec Parental Insurance Plan (QPIP), all residents of all other provinces should contact Employment Insurance.

It’s time to select a benefit plan! Either under Employment Insurance or the Quebec Parental Insurance Plan. Please refer to your collective agreement for more details on the eligibility of each plan.

Your Public Service Health Care Plan coverage continues automatically while you are on leave unless you choose to opt out of the plan during your leave. Premiums owed will be recovered over a period of time when you return to work. You must indicate your choice in the selected section.
