Understanding report type codes
The three possible report type codes are as follows:
- O = Original
- A = Amendment
- C = Cancelled
Amending individual records
How to amend individual records
After filing your T4FHSA annual information return, you may notice an error on a T4FHSA individual record. If so, you will have to prepare an amended individual record to correct the information.
If an original or amended record has already been submitted to the Canada Revenue Agency (CRA), and information on that record needs to be amended, an amended version of that individual record must be sent to the CRA. An individual record that is rejected remains in the CRA's database and a corrected individual record is required.
When submitting an amended individual record, all of the original data on the previous individual record that remains unchanged as well as the modified data must be submitted. If a transaction was reported in error and the transaction never took place, it can be omitted from the amended record. Use slip report type code "A" and summary report type code "A". An amended return can contain amended or cancelled records, but it cannot contain original records.

When to amend individual records
Amended records should be prepared to correct the following:
- Reporting errors such as misreporting a transaction value or transaction type
- Incorrect identification information such as given name, family name, date of birth or date of death
- Election established under the incorrect social insurance number (SIN) or temporary tax number (TTN), which needs to be established under the correct SIN or TTN
- Original records where the recipient’s date of death was not provided because the issuer was not notified about the death until after they had submitted the original record
Incorrect amount reported on the individual record
Example 1
Zain contributed $4,000 to his FHSA on May 26, 2023. He also transferred $1,000 from his RRSP to his FHSA on August 23, 2023.
The issuer filed Zain’s record with an incorrect contribution amount of $3,000. The issuer will need to submit an amended record for Zain to report the correct amount:

Example 2
Isru did not make any contributions to her FHSA in 2023. However, the issuer filed Isru’s record with an incorrect contribution transaction of $5,000 on June 10, 2023.
Since Isru did not make any contributions, a contribution transaction should not have been reported for her. The issuer will need to submit an amended record for Isru to remove the incorrect contribution transaction:
If a transaction was reported in error, it can be omitted from the amended record entirely.

Incorrect transaction type reported
Example 1
Mia transferred $3,000 from her RRSP to her FHSA on June 4, 2023. The issuer made an error and reported this transaction as a contribution instead of an RRSP transfer in. The issuer will need to submit an amended record to identify the correct transaction type:

Change in account ownership
Example 1
Kymani and Natasha are common-law partners and they both opened an FHSA with the same issuer in 2023. Natasha's SIN is xxxxxx505, while Kymani's SIN is xxxxxx055. Natasha made a contribution of $5,000 to her FHSA on June 7, 2023.
While completing the T4FHSA annual information return, the issuer made an error on Natasha's record and reported the incorrect SIN. The issuer reported Kymani's SIN under Natasha's FHSA contract number.
Since the FHSA belongs to Natasha, the issuer will need to submit an amended record to change the account ownership and identify the correct SIN:

Cancelling individual records
How to cancel individual records
After filing your T4FHSA annual information return, you may need to completely remove an individual record. If so, you will have to submit a cancelled individual record for that contract number to remove the information for that tax year. When submitting a cancelled individual record, all the data on the previous individual record (original or amendment) must be included. Use slip report type code “C” and summary report type code “A”.
Do not use slip report type code “C” when:
- an amended record can correct the mistake in the previous record
- you want to cancel previously submitted and valid records in order to refile them with other records that were not previously submitted (the records that were not previously submitted should be sent in a separate file as an original. For more information, go to Adding individual records).
Use an amended record for any other data element you want to modify.
When to cancel individual records
A cancelled record applies to a particular reporting year. When a cancelled record is received for a particular year, the CRA will eliminate the entire record.
Use slip report type code “C” for the following scenarios:
- You need to correct an FHSA contract number or FHSA identification number previously submitted in error
- A record was filed that the CRA cannot accept as valid. A cancelled record should be filed to acknowledge that this contract is not an FHSA
- Upon contacting the FHSA Processing Unit, it was determined that a cancelled record is required
Incorrect FHSA contract number reported
Example
Nia opened an FHSA on October 1, 2023. She was assigned FHSA contract number xxx152xxx and she made a contribution of $6,500 on November 5, 2023.
While completing the T4FHSA annual information return, the issuer made an error in Nia's record and reported the incorrect FHSA contract number.
The issuer will need to submit a cancelled record to remove the election from the incorrect FHSA contract number.

Adding individual records
How to file additional individual records
After you file your T4FHSA annual information return, you may discover that you need to send the CRA additional T4FHSA records. If you have original records that were not filed with your information return, file them in a separate original return. Do not combine the new records with previously submitted ones.
Any additional slips that are filed after the due date may result in a penalty.
