The SIR reporting method to be used is in accordance with DAOD 2008-3. This SIR format is recreated below with additional amplifying fields required for OP HONOUR HISB reporting.

Date, time and location of the significant incident.

This concerns the complaint, incident or occurrence. If relevant, this para concerns the date the incident was resolved or closed.

Name, location and telephone number of the person or agency initially reporting the incident.

Bearing in mind the privacy requirements in para 6-9 of Annex B, as appropriate include Rank, Unit, Element, Environment and Gender of the individual reporting HISB incident (victim, bystander, chain of command, etc.)

Who or what was involved, what happened and how it happened?

Bearing in mind the privacy requirements in para 6-9 of Annex B, as appropriate include Rank, Unit, Element, Environment and Gender of the individual both the victim and perpetrator or alleged perpetrator, where the HISB occurrence took place, and include details of the offence/alleged offence/ complaint.

A non-exhaustive list of location/circumstances is as follows:

  • on military base/wing;
  • on a ship;
  • in a training area/field exercise;
  • while on TD/Attached Posting;
  • on a deployed operation;
  • on a training course;
  • during a CAF-sanctioned function;
  • at a military college;
  • in married or single quarters;
  • in a personal residence;
  • civilian establishment; or
  • other (specify)

A non-exhaustive list of offences/alleged offences/complaints including appropriate detail as follows:

  • Abuse of power
  • Sexual interference;
  • Sexual exploitation;
  • Sexual assault; or
  • Offence/alleged offence or complaint with an OP HONOUR nexus.

Name of parent unit and specific information concerning the persons and equipment involved.

Bearing in mind the privacy requirements in para 6-9 of Annex B, any additional information not captured in para 3.

Any possible broader implications of the incident, including the effect on future operations.

Results/actions taken by Chain of Command. A non-exhaustive list of examples includes:

  • Individual is currently held by MPU XXXXX and the victim is being treated by medical services. CFNIS is investigating. PAO is tracking and preparing MRLs;
  • Individual is currently held in custody by Local or National Police and the victim is being treated by medical services. MP/CFNIS are tracking.
  • Unit investigation with MP support on going. The victim has been offered Victim Services (CF MP Group);
  • The chain of command is currently investigating the incident;
  • The incident was reported by the chain of command to the MP and investigation will be carried out;
  • The chain of command has notified the Military Police and the investigation has been transferred to the RCMP;
  • The chain of command is aware and has removed the alleged offender from its position of authority and temporarily assigned him/her to administrative duties until an investigation is completed;
  • The chain of command has taken administrative action (IC/RW/C&P); or
  • A previously reported incident has been investigated and was determined to be unfounded and closed.
