Maternity and parental leave and benefits

2. Submit a maternity or parental leave application

Maternity leave can begin before or on the week the child is born. However, members are eligible for maternity leave up to the maximum duration prescribed under the Employment Insurance Act and Act Respecting Parental Insurance and up to 14 days of deemed days IAW QR&O 16.26(5) (Clarification). Parental leave can begin on or after the day the child is born or the day the child comes into member’s care.

How to apply

  1. At least six weeks before you plan to start your leave, contact your Orderly Room (OR) and complete these forms:
    • Application Form – Maternity/Parental Leave and Allowances (DND 2268)
    • CF Leave Request / Authorization (CF 100)

  2. Understand your eligibility and options for maternity/parental benefits through EI or QPIP. In addition, set up your My Service Canada account for EI benefits or clicSÉQUR user code and password for QPIP benefits.

  3. After your Employment Insurance (EI) or Quebec Parental Insurance Plan (QPIP) application is approved, complete the summary of your EI or QPIP benefits, including the gross benefit rate and waiting period information (for EI only).

  4. Submit the completed forms to your OR. 

  5. Contact your OR as well as the Pension Centre for additional information regarding the impact of this LWOP or ED&T on your pension and Supplementary Death Benefit.

  6. Update your contact information, including your personal email address with your OR to allow the CAF to contact you at home if there are of any administrative or pay issues.
