Report on your project

This section lists the mandatory forms you must provide to successfully complete your CSJ project.

Employer and Employee Declaration form

You must submit an Employer and Employee Declaration (EED) form for each participant you hire to validate their eligibility. Complete this form within 7 calendar days of the start of the CSJ-funded employment. You will find the link to the EED form in your approval email. If you cannot complete this form online, contact us for mailing instructions.

Note: We cannot issue any advance or payment until we receive the form and confirm the youth's eligibility.

We may terminate your project if you do not submit this form.

Employee Consent form

If you intend to submit the Employer and Employee Declaration (EED) online, participants must complete the Employee Consent form. Once each participant has completed the form, you can submit the EED through Grants and Contributions Online Services (GCOS) or by using the online fillable form.

The Employee Consent form collects the information you need to complete the mandatory EED online. The employee must answer all questions, sign, and date the form for it to be complete. When you complete the EED, you must enter the employee's contact and personal information exactly as it appears on the consent form.

You must keep a copy of the signed Employee Consent form in your files for 6 years, and we may verify it during project monitoring. You do not need to send us a copy unless we request it.

You may complete the form on the employee's behalf, as long as the employee reviews the information, completes the declaration, and signs the form to confirm their consent to release their personal information to Employment and Social Development Canada (ESDC).

Payment Claim and Activity Report

This form details your project costs and activities. You must submit it within 30 days after the last CSJ-funded employee finishes their job, even if they continue working for you beyond the CSJ project.

Note: You must complete the employer questionnaire before you can submit a Payment Claim and Activity Report to get reimbursed for eligible project costs.

If you cannot complete this form online, contact us for mailing instructions.

If we do not receive your report within 30 days, we may not reimburse you and may terminate your project. We may also consider any funds already advanced as an overpayment that you must repay.

Reimbursement rules

We can only reimburse the costs listed in your agreement. To verify what's covered, refer to the Wage subsidies available section on our webpage. If you're still unsure about what's included, please contact us.

Mandatory Employment Related Costs (MERCs)

By law, you must remit MERCs on behalf of youth participants.

Employer questionnaire

You must complete the employer questionnaire to share your feedback. Complete this before you submit your Payment Claim and Activity Report. Once you finish the questionnaire, you will receive a confirmation number that you must include in your Payment Claim and Activity Report.
