4. Develop your application

It is recommended that you consult this section while filling in the Grant Application Package, as it provides important details on the information and supporting documentation ESDC requires to assess your application. Provide your answers on the Grant Application Package and attach only the documents requested (see Apply).

All required documents must be attached, otherwise your application will be considered incomplete. You are required to submit your application in one complete package once the reservist has returned from active military activity (including any pre-deployment training deployment and post-deployment activities that are mandatory, including leave).

How to fill out your Grant Application Package

The Grant Application Package consists of the following sections:

Section A – Program information

This section provides a description of Compensation for Employers of Reservists Program (CERP) including all relevant information and support for the applicant.

Section B – Notice to applicants

This section provides the applicant with information about the collection of personal information.

Section C – Grant application for funding under CERP:

Section D1 – Articles of agreement

This section provides all contract information between Canada and the applicant organization once the submitted Grant Application Package is approved by the Department of National Defence.

Section D2 – Declarations and signatures

This section provides a list of declarations that the applicant must certify to prior to submitting their Grant Application Package.

Section D3 – Canada approval

This section is to be completed by Canada.

Section C – Grant application for funding under CERP

Part 1 – Organization

1A – Organization identification (For employers of reservists and self-employed reservists)

ESDC uses the information you provide in this section to establish your organization's identity.

Question 1 – Legal name

Please provide the legal name of your organization. Usually, this is the name associated with your registration with the Canada Revenue Agency (CRA), or the name that would appear on funding cheques.

Question 2 – Operating (Common) name (if different from legal name)

Please provide the operating (or common) name of your organization if it is different from the legal name.

Question 3 – Business number

Please provide your 15-digit CRA business number (for registered charities and not-for-profit organizations, your registration number). You must provide a document that includes your business or registration number (e.g. tax related documents, CRA correspondence, etc.).

Question 4 – Organization type

Your answer to this question must be one of the following:

Employer of a reservist

Self-employed reservist

Refer to Eligibility.

Question 5 – Year established

Please indicate the year your organization was established.

Questions 6 to 10 – Organization address

Please indicate the address of your organization – Applicants must provide a complete address in a format recognized by Canada Post. Information is available on the Canada Post website.

Questions 11 and 12 – Telephone and fax

Please indicate the telephone and fax numbers of the organization.

Question 13 – E-mail address

Please indicate the e-mail address of the organization.

Questions 14 to 18 – Mailing address

Please indicate the mailing address if different from your organization's address.

Questions 19 and 20 – Telephone and fax

Please indicate the telephone and fax number if different from your organization's address.

1B – Organization contact

Please provide a main contact for the purposes of communicating with your organization about this grant. This could be the business owner/manager or a delegate of their choice.

Questions 21 and 22 – Name

Please provide the name of the person in your organization who will be Employment and Social Development Canada's (ESDC) main contact for the grant.

Question 23 – Position title

Please provide the title of the organization contact identified in Questions 22.

Example: Business owner, President, Executive Director.

Question 24 – Preferred language of communication

Please indicate the preferred language of the organization's main contact for both written and spoken communication.

Question 25 – Organization contact – Address

Please indicate whether the address for the Organization Contact is the same address as the Organization Address or the Organization Mailing Address given in Section 1A. If different, please indicate in the box.

Questions 26 to 30 – Contact address

If you selected "Different" in Question 26, please provide the address for the Organization Contact here.

Questions 31 and 32 – Telephone and fax

Please indicate the telephone and fax numbers of the Organization Contact.

Question 33 – E-mail address

Please indicate the e-mail address of the Organization Contact. Please note that this is where all correspondence will be sent.

1C – Additional organization information

Question 34 – Does your organization owe any amounts to the Government of Canada?

If yes, please indicate the amounts owing in the spaces provided. See the following example:

Question 35 – Is a payment plan in place?

Please indicate whether or not payment arrangements have been made.

Answering "yes" to this question will not invalidate your application. However, you must provide details as to the type of debt owed and the arrangements that have been made for repayment.

Part 2 – Reservist, mission or training, and employment information

ESDC uses this information to determine if the reservist, the mission or training and the employment situation meet all of the program eligibility requirements.

2A – Reservist information

Question 36 – Given name(s)

Please provide the given name of the reservist.

Question 37 – Surname

Please provide the surname of the reservist.

Question 38 – Military service number

Please provide the reservist's military service number.

2B – Mission or training information

Question 39 – Mission or training name

Please provide the name of the Chief of Defence Staff directed operation for which the reservist is taking a leave of absence from work.

Question 40 – Start date of military activity

Please provide the date the reservist began his/her active military activity.

Question 41 – End date of military activity

Please provide the date the reservist was released from active military activity. This represents the date when the reservist was deemed to have completed all the requirements of his/her mission or training. In order for your organization to be eligible for compensation under this program, the leave of absence for active military activity must have a minimum duration of 30 days.

2C – Employment information

Please answer questions 42 and 43 if you are applying as an employer of a reservist.

Please answer question 44 if you are applying as a self-employed reservist.

Question 42 – Date the reservist started working for your organization

Please provide the date the reservist started working for your organization. In order for your organization to be eligible to receive compensation under this program, the reservist must have worked for your organization for a minimum of 13 consecutive weeks (3 consecutive months) prior to the leave of absence for active military activity.

Question 43 – Average number of hours per week the reservist works for your organization

Please provide the average number of hours per week the reservist works for your organization. You must provide copies of the reservist's pay stubs for the 13 consecutive weeks (3 consecutive months) prior to the leave of absence for active military activity with your completed Grant Application Package. The information contained in the pay-stubs submitted with your application will be used to confirm the average number of hours indicated under this question.

In order for your organization to be eligible to receive compensation under this program, the reservist must have worked an average of 30 hours or more per week for your organization over 13 consecutive weeks (3 consecutive months) prior to the leave of absence for active military activity.

Question 44 – Your business currently represents and it has represented your main source of employment/income for at least 13 consecutive weeks (3 consecutive months) prior to the leave of absence for active military activity.

In order to be eligible to receive compensation under this program, self-employed reservists must demonstrate that the self-employment has represented their main source of employment/income for a period of at least 13 consecutive weeks (3 consecutive months) prior to the leave of absence for active military activity. You must provide a copy of your most recent financial statements signed by a certified accountant with your completed Grant Application Package (which includes the 13 consecutive weeks (3 consecutive months) prior to the leave of absence for active military activity).

2D – Checklist and supporting documents

Please ensure to include all the additional required documents with your Grant Application Package.

Part 3 – Annex (for Quebec applicants only)

To be completed by organizations located in Quebec: Please note sections 3.11 and 3.12 of the Act respecting the ministère du Conseil exécutif (R.S.Q.c. M-30) and ensure to answer questions 1 and 2 of this part in the Grant Application Package.

Section D1 – Articles of agreement

Once completed and submitted, this CERP Grant Application Package, if approved by Employment and Social Development Canada, will form the agreement between Canada and the organization. The organization will then be subject to the Articles of Agreement included in the Grant Application Package.

Section D2 – Declarations and signatures

Please ensure that your application is signed by an official, authorized representative of your organization. People with signing authority are normally one or more of the executive members of the board of directors (President, Vice President, Secretary or Treasurer) and employees of the organization (Chief Executive Officer, Executive Director, Chiefs of Finance or Human Resources).

The Grant Application Package must be signed in accordance with the organization's statutes, by-laws or other constituting documents. For example, the president and the chief financial officer may be required to sign all outgoing documents.

Section D3 – Canada approval

You should not assume any commitment on the part of Department of National Defence (DND) or ESDC (referred to as «Canada») until funding has been approved and the Approval page of the Grant Application Package has been signed by a representative of Canada. ESDC will notify you in writing of the outcome of the review of your application.

Notice to applicants

The information you provide in this Grant Application Package is collected under the authority of the Department of Employment and Social Development Act for the purpose of assessing eligibility of your application.

Participation in CERP is voluntary. If you do not provide all of the personal information required in this application, your application will not be processed.

As part of the assessment process and issuance of the grant, information may be shared with the Department of National Defence, Public Services and Procurement, and Members of Parliament.

The information you provide may also be used and/or disclosed for policy analysis, research, and/or evaluation purposes. However, these additional uses and/or disclosures of your personal information will never result in an administrative decision being made about your application.

Your personal information is administered in accordance with the Department of Employment and Social Development Act, the Privacy Act and other applicable laws. You have the right to the protection of, and access to, your personal information, which is described in Personal Information Bank ESDC PPU 721. Instructions for obtaining this information are outlined in the government publication entitled Info Source. Info Source may also be accessed on-line at any Service Canada Centre.
