Using PDF forms
General information
The Canada Revenue Agency (CRA) produces forms and publications in Portable Document Format (PDF), which are fillable and savable for your convenience. To make sure you achieve the best possible results while using our forms, below you will find some information to assist you.
Filling and saving PDF forms
Certain browsers offer a built-in PDF viewer that does not support the CRA’s fillable/savable PDF forms. To be able to fill in and save a PDF form, download and then open the form using the free Adobe Reader:
- To download the fillable/savable PDF, right click on the file name and select “Save link as”.
- Save the PDF form in a folder that you will easily find on your computer.
- Launch Adobe Reader.
- Select “File”, then “Open…”, and browse to the folder where you saved the file. Select the file, and click “Open”. You can now fill and save your PDF form.
Note: CRA fillable/savable PDF forms have been compatibility tested with Adobe Reader 10 and higher and may not function optimally in other PDF viewers. The CRA does not recommend using mobile devices with our PDF forms. Adobe support for Acrobat Reader System Requirements.
Printing PDF forms
When you are ready to print a PDF, from the Adobe Reader “Print” dialog box, make sure you have checked the “Choose paper source by PDF page size” checkbox and specified the scaling options for the size of paper you are printing on.
Need a signature? If the form requires a signature, you must print the completed form and sign by hand.
- Use the latest printer driver: Printer manufactures regularly release updated printer drivers to improve compatibility with newer software. Make sure you have the latest version.
- Print to a different printer: If you have a second printer connected to your computer, try printing the file to it.
- Use the "Print as image" option: Adobe Reader gives you the option to print the PDF form as an image and this can resolve certain printing issues. You can find this setting in the “Advanced” section of the “Print” dialog box in Adobe Reader.
Remember: You are responsible for the completeness and accuracy of any information submitted on a form. Be sure to double-check all data entered.
NR4
The PDF fillable form lets you complete a page with information for only one recipient. The page contains two slips.
Non-resident recipient's copies
Print one page containing two slips for the recipient. Make sure your account number is in the box called “Non-resident account number” on these copies.
Canada Revenue Agency's copy
Print one page containing two slips for the Canada Revenue Agency. Cross out the second slip completely. Do not separate the slips—attach the full page with your Summary. Do not enter the information from the slip you crossed out on your Summary. Only enter the information from the slip you did not cross out. Make sure your account number is in the box called “Non-resident account number.”
Payer or agent's copy
Keep an electronic or printed copy of the slip you give to each non-resident recipient for your records. Make sure your account number is in the box called “Non-resident account number” on those copies. For printed copies it is good to cross out the second slip completely.
T1204
The PDF fillable form lets you complete a page with information for only one recipient. The page contains two slips.
Recipient's copies
Print one page containing two slips for the recipient. Do not enter your account number in the box called “Payer's account number” on these copies.
Canada Revenue Agency's copy
The Canada Revenue Agency only accepts T1204 forms that are filed electronically. For more information, see Filing Information Returns Electronically (T4/T5 and other types of returns).
Payer's copy
Keep an electronic or printed copy of the slip you give to each recipient for your records. Make sure your account number is in the box called “Payer's account number” on those copies. For printed copies it is good to cross out the second slip completely.
T3
The PDF fillable form lets you complete a page with information for only one recipient. The page contains three slips.
Recipient's copies

Canada Revenue Agency's copy
Print one page containing three slips for the Canada Revenue Agency. Cross out the second and third slips completely. Do not separate the slips—attach the full page with your Summary. T4

Print one page containing two slips for the employee. Do not enter your employer's account number in box 54 on these copies.

Canada Revenue Agency's copy
The Canada Revenue Agency only accepts T4E or T4E(Q) forms that are filed electronically. For more information, see Filing Information Returns Electronically (T4/T5 and other types of returns).

T4PS

The PDF fillable form lets you complete a page with information for only one partner. The page contains two slips.
Partners' copies
Print one page containing two slips for the one partner. Do not enter your partnership account number in box 001 on these copies.
Canada Revenue Agency's copy
Print one page containing two slips for the Canada Revenue Agency. Cross out the second slip completely. Do not separate the slips—attach the full page with your Summary. Do not enter the information from the slip you crossed out on your Summary. Only enter the information from the slip you did not cross out. Make sure your partnership account number is in box 001.
Filer's copy
Keep an electronic or printed copy of the slip you give to each partner for your records. Make sure your partnership account number is in box 001 on those copies. For printed copies it is good to cross out the second slip completely.
T5018
The PDF fillable form lets you complete a page with information for only one recipient. The page contains two slips.
Recipient's copies
Print one page containing two slips for the recipient. Do not enter your account number in the box called “Payer's account number” on these copies.
