How to apply

To apply to have your student loan cancelled, download and complete the application form (PDF format, 181 kb) and the medical report (PDF format, 737 kb). Have a doctor or nurse complete the medical report. Mail both signed original documents with any supporting documents to the Canada Student Financial Assistance Program (CSFA Program) at the following address:
