Providing medical support: the Canadian Armed Forces Transition Group (CAF TG)

The CAF TG works in cooperation with key partners to provide medical support, recovery, and rehabilitation services to you as a Canadian Armed Forces (CAF) member so that you may return to duty. The Group also supports military families like yours as well as former CAF members and the families of deceased CAF members.

Being posted to the CAF TG

Depending on your medical situation, your commanding officer may request that you be posted to the CAF TG according to the CAF Military Personnel Instruction 04/22 – Posting Regular Force (Reg F) members to the Canadian Armed Forces Transition Group (CAF TG).

These provisions require that your commanding officer submit your request to your base or wing surgeon, Career Manager Authority, and local transition unit using the form DND 2794, Request for Posting to Canadian Armed Forces Transition Group (CAF TG) – (Regular Force Supported Member). Upon completion of their review, these parties will submit your request to your career manager who, after further review, will submit it to the CAF TG for a decision. It is the Comd CAF TG/DGMT or a delegated authority who has the ultimate authority for your posting to the CAF TG.

Your release from the CAF TG

The primary goal of the CAF TG is to support CAF members throughout their recovery and rehabilitation so that they may return to duty. However, some members may eventually be released for medical reasons.

While you are posted to the CAF TG, the Career Manager for ill and injured will be responsible for administering all career management actions that require an Administrative Review. This includes release waivers, promotions, and rank reversion. If you have questions about the case management process, contact your Career Manager for ill and injured.

Grieving a decision

If you are unsatisfied with a medical posting or release decision and if you believe your application was not processed fairly, you may file a grievance. Consult a Conflict and Complaint Management Services (CCMS) Centre agent for information about the grievance process.

If your grievance is denied at the Initial Authority level, you may submit it to the Final Authority. For more information, contact your chain of command for more Information.
